Step 1 - Confirm Insurance

We are accepting new patients with these insurance plans:

• Anthem / Blue Cross
• Cigna
• Denver Health Highpoint
• Kaiser HMO & PPO Plans
• MotivHealth
• UnitedHealthcare
• UMR

We are NOT accepting new patients with: Aetna, CHP+, Medicaid, Self-Pay or Tricare.

If you are expecting, we are happy to set up a pre-natal visit to get to know you before the big event!

As a medical home dedicated to following pediatric best practices, we ask all patients to be fully vaccinated and stay current on well care exams.  

Step 3 - Transfer Records

If you have a prior primary care provider, send this form to them to request that medical records for your child(ren) are sent to us.  If possible, please request the records a week before to your first visit.  Have them sent to:

Step 4 - 1st Visit Prep 

If your insurance requires a Primary Care Physician, please designate us as your PCP.

We'll use Spruce to send a reminder before your visit, along with any forms that need to be competed.  Forms can also be returned within the Spruce app.
